Leadership expands with appointment of chief medical officer and

These “Heroes” will form the foundation of the HomeHero Collaborative, a
new care management platform for hospitals and health plans that
connects and extends the health system into the home. The platform

patient health data, and then uses an analytics engine to refer patients
to the appropriate care provider within the Collaborative, helping
