Using a palliative care population as a case study, the session will
explore strategies for partnering with local clinicians that have
specialized expertise in providing services that are valued by members
and caregivers: symptom management, nutritional counseling, health
system navigation, coordination with physicians, access to care and
spiritual support.

The goal is to promote supportive environments during care transitions,
augment centralized care management resources, and streamline access to
care at the local level and in support of the medical home and primary
care physician.
